In order to be eligible for a card through our practice you must be a member of our Pear’d program.
This program costs $100 per member per month, provides our patients with unlimited Primary Care without c0-pays, co-insurance, or bills due after the time of service.
Cardholders must remain active in the Pear’d Program and must remain current or card approval will be rescinded.
The process requires an initial evaluation with our practitioners to determine your eligibility. This does require an office visit and is non refundable.
At the discretion of our practitioners a decision will be made to approve or not approve the patient for a card.
We no longer accept insurance...
As of December 1, 2022, our clinic is no longer accepting insurance for patient visits. Instead, we are now offering two unique services:
Our Primary Care service, Pear’d, is ideal if you are taking daily, routine medications, have health conditions that require regularly scheduled visits, or if you want a provider who knows about your health needs and can provide you with preventative care and health counseling on a regular basis.
Joining our Pear’d program provides all inclusive $100/month unlimited primary and urgent care with no co-pays, deductibles or mystery bills after the time of service.
Our Urgent Care service is available for walk in or scheduled urgent care visits for $150.00 per visit. This service is for those who have a primary care provider but need a visit for an unexpected health problem. We are happy to refill medications at these visits but refills are limited to a one month supply. We can also provide you with an annual exam and laboratory testing through our urgent care service for an added charge.
Our fees are lean and straightforward.
