We have RAPID Covid-19 testing through Quidel Sophia technology.

  • We are testing on a first come first serve basis as walk-up testing – NO APPOINTMENTS.
    9:30 a.m. and 4 p.m.
  • We do not test children younger than 6 months.
  • We will not accept calls for COVID testing questions. All information is here on the website. We are unable to handle the call volume. Please read instructions carefully.
QUICKEST WAY TO TESTING: (cash pay, walk up instructions)
This form is NOT a testing appointment or confirmation form. It is only a payment form with instructions for receiving your test results.
  1. Cash pay here. Bring your emailed receipt – If you don’t see it, check your spam folder. It will come from info@hucfm.com
  2. Print, fill out and bring these forms.
  3. Walk up directly to testing window. There is no reason to enter the clinic.
  4. If you are a new patient you must register on our PATIENT PORTAL in order to receive test results. A computer is required to register. Registration will not work on a phone.

Come into the clinic for insurance verification with the receptionist BEFORE heading to the testing window.

Receiving Your Results

If you are not a current client you MUST register with The Athena Electronic Medical Record.

A computer is required to register. Registration will not work with a phone. 

All results will be reported through your Athena Patient Portal.

If you are positive you will receive a call within an hour.

If you are negative your results will be available by the end of the day within the Athena Portal. To view your results you must use a PC or Laptop, click on Patient Portal then go to My Health and click Medical Forms (NOT TEST RESULTS).
